WIAA Student Athlete Physical Exam Form (Spanish) (Hmong)
Students registering for athletics will need to upload a current sports physical form to the activity registration portal. We do have sports physicals on file from last year that we can upload if they are still valid. A physical exam taken April 1 and after is valid for the following two school years; a physical exam taken before April 1 is valid only for the remainder of that school year and the following school year. Bellin FastCare locations in Green Bay are currently offering sport physical walk ups for $20.
